Cite a California court rule in CSM format

A rules-of-court citation gives the title of the rules, the word “rule,” and the number — and never the designation “subdivision” or “subd.” Subdivision letters attach directly to the rule number (CSM §2:18). The builder emits the parenthetical form with subparts joined by an ampersand and the running-text form as the manual prints them.
